I really don't have the time to customize my SmugMug site, what all services/people are out there to customize my site for me? Ideally the service would have a list of sample sites

Please don't reply with 'I'll do it!', I'm looking for a list here hopefully with a range of pricing and options.

This list is intended to be a shared resource that everyone can use and update. Please check for an existing answer for the customizer you want to suggest first. You can upvote those you are happy with, talk about each in the comments on the answer, etc. Please feel free to add more examples of their work to each entry.

I converted this to a community wiki answer and divided up the answers to allow others to edit, and so that users can vote for the best customization companies, and leave comments for the individual companies instead of lumping it all on one answer. – Doc Walker♦ Feb 11 '12 at 13:29
